Referring Doctors

If you would like to refer a patient you may like to use our referral form.

Under the Federal Governments Allied Health Initiative, GPs can refer patients with a chronic medical condition to dietitians.

There is a limit of 5 services from all Allied Health Providers per calendar year.

Patients are considered eligible if their GP has completed the following prerequisite Chronic Disease Management items in the previous 2 years

  • GP Management Plan (GPMP) – Item 721 (or item 732 for a review of a GPMP)
  • Team Care Arrangement (TCAs) Item 723 (or item 732 for a review of TCAs)

For patients that are permanent residents of an Aged Care facility the GP must have contributed to a multidisciplinary team care plan prepared by the Aged Care Facility (item 731).

A copy of the TCA and Referral form for Allied Health Professionals, indicating the number of visits allocated for dietetic services, is required at the consultation.
